Barry Pointe Family Care is proud to announce we successfully re-certified as a Patient-Centered Medical Home. The National Committee for Quality Assurance. The Patient-Centered Medical Home is a health care model that focuses on the highest quality care delivered in a patient-centered atmosphere. The principles of this model involve organizing care around patients, working in teams, and coordinating and tracking care over time.
