New Patients- First Visit

Please arrive at least 20-30 minutes prior to your appointment to allow time to collect your insurance and personal information.  Bring your insurance card, photo identification and completed New Patient paperwork.

If you take medications, vitamins or supplements, please bring a list of those with dosage(s).  Current immunization records are also required for children.

If you are not able to keep you scheduled appointment please call to cancel or reschedule.  We charge for no shows.  We will send an appointment reminder via text message.  It will indicate to check in 15 minutes prior to your appointment, but that does not apply to New Patients.

Insurance

We accept all insurance except Medicaid and new Medicare patients. Established patients who become eligible for Medicare will continue to be seen in our practice. Please click here for more information.
