Barry Pointe Family Care recently received recognition as a Level 3 Patient Centered Medical Home by the National Committee for Quality Assurance (NCQA). This reflects what we've known all along, that BPFC is committed to providing the highest quality care to its patients.
"The Patient Centered Medical Home promises to improve health and health care," said NCQA President Margaret E. O'Kane. "The active, ongoing relationship between a patient and a clinician in a medical home fosters an all-too-rare goal in care: staying healthy and preventing illness in the first place. PPC-PCMH Recognition shows that Barry Pointe Family Care has tools, systems and resources to provide their patient with the right care at the right time."
What does PCMH mean for you as a patient?
Becoming a Patient Centered Medical Home shows that BPFC provides high quality, patient-centered care. You'll see this when:
- Your doctor works with you as an important member of your health care team to manage your care. This means helping you set and reach your health goals.
- You have same-day access to the practice when you're sick, but also when you are well. That means our doctor will also track your status even when you are healthy.
- Your BPFC health care team will work to help you stay out of the emergency room by showing you how to manage your health conditions such as diabetes.
- Your PMCH practice knows you.
Having PCMH recognition shows that Barry Pointe Family Care is your home for health. Providing high-quality, patient-centered care is something we've been committed to since the day we first opened our doors. Now we have national recognition that what we're doing is working!
